Roxie does not have to roll to remove the arrow. She does, however, have to roll Dex + Medicine to do so without causing another point of Lethal damage.

Roxie manages to get the arrow out without causing further significant damage to muscle or bone. Just when she has completed the process, the arrow starts to smoulder, the paper bursts into flame... then suddenly the thick, middle section of the arrow -the one that had just a few seconds earlier been pressed against her abdomen - ignites with hot blue-white flame. Roxie feels the flesh of her hand burn to the bone.
